Repair Services for Fiber Optic Splice and Test Equipment

To initiate a repair order:

  1. Call your Corning Cable Systems Customer Service Representative at 1.800.743.2671 (U.S. & Canada) or 1.828.901.5000 (International).

  2. Give the Customer Service Representative your company name and state or region from which you are calling.

  3. Provide the Customer Service Representative with:
    - Equipment name and model number
    - Serial number
    - Date of purchase
    - Where purchased
    - Description of the problem
    - The name and phone number of a contact whom our technicians can call for further details

  4. Make payment arrangements for the repair.

    To pay by credit card provide:
    - Credit card number and expiration date
    - Name of the credit card holder, exactly as it appears on the credit card
    - Telephone number of the credit card holder

    To arrange payment with a formal purchase order:
    - For existing accounts, provide your Corning Cable Systems account information
    - For new accounts, provide references and complete a Corning Cable Systems credit
      application
